Annual Wellness Visit

It’s important to see your primary care provider (PCP) at least once every year for a wellness visit to develop a personalized prevention plan, even if you’re not sick. This is a good time to build a relationship with your PCP so that they will know your health history when you need to see them for sickness or other problems.

What can I expect at the Annual Wellness Visit?

The Annual Wellness Visit is time with your PCP to develop or update a personalized prevention plan based on your current health and risk factors. It also includes a review of your health, as well as education and counseling about the preventive services you need including: certain screenings, shots, possible medications, and referrals for other care, if needed.

It can also include:

• A review of your medical and family history

• Updating a list of current providers and prescriptions

• Height, weight, blood pressure, and other routine measurements

• Detection of any cognitive impairment and functional status

• A pain assessment

• Personalized health advice

• A screening schedule for appropriate preventive services

• Advanced Care Planning

Who should I see for the Annual Wellness Visit?
